NHS Members: print the above form, complete it, have the adult supervisor of your service sign it, and turn in the form for individual community service credit.  
  • It's best to do this as you work or immediately after your service, not several months after you have finished.
  • If all 20 hours are completed at the same place, you can use one form. Just list all the dates and times, and get the form signed when you complete your hours.
  • If you complete your hours at several places, please use a separate form for each place and get each signed as you finish there.


Rules:
  • You need 20 hours before March 31, 2020.
  • Your form needs to be signed by a non-family member.
  • You cannot get paid for what you do! This needs to be from the goodness of your heart. :)
